The Extended Wellness Program is a unique after-care service designed to reinforce long-term well-being, give you space to reflect on your progress, and ensure you have a safety net of structured support as you transition out of active treatment.

This program is meant to help you stay connected to your growth journey without returning to therapy. Instead, it provides a supportive bridge — keeping you grounded in the progress you’ve made and confident in the skills you’re applying day-to-day.

At Centre Wellness, we acknowledge that healing doesn’t stop when therapy ends. That’s why we’ve created the Extended Wellness Program. 

The Extended Wellness Program is Designed to Support Clients in:

Increasing confidence in independently applying strategies
Reducing risk of setbacks or relapse
Having accountability to stay aligned with wellness goals 
Enhancing overall well-being through intentional after-care


Staying connected to their growth after therapy
Strengthening resilience during every day stressors
Maintaining long-term treatment gains 
Feeling supported through transition out of active therapy 

3. Participate in Structured Check-ins

How Does it Work?

Your sessions are scheduled at 2, 4, and 6 months after therapy ends. Each check-in has a clear purpose to help you maintain your progress. These sessions are designed to help you reflect, stay connected, and continue building confidence in your growth.

1. Complete Therapy with Primary Therapist

Clients first complete their course of therapy with their therapist at Centre Wellness. If eligible, your therapist will introduce the Extended Wellness Program as a way to stay connected and supported after therapy. They will complete a brief referral form on your behalf if you would like to participate.

2. Enter the Extended WEllness program

Once your referral is accepted,  you will be enrolled in the program at no cost. The program includes three complimentary sessions spread out over six months. These sessions are facilitated by a supervised student therapist (doctoral or master’s level). Our admin team will connect with you to confirm enrolment and schedule your sessions.

What to Expect

Through purposeful sessions centered on your long-term wellness, you will be provided with dedicated time to reflect on the growth you’ve made since therapy. In these sessions, you can expect:

Guided reflection exercises to track progress
Reviewing how coping tools are working day-to-day
Practicing and strengthening key strategies together
Using guided meditations or grounding activities 
Exploring challenges in a supportive space
Identifying early warning signs and creating proactive plans 
Celebrating wins and reinforcing confidence
Planning next steps and setting clear wellness intentions
